Losing power when going up hill.

Hi, I have a 2005 Megane Scenic II 1.5DCi with 73k on the clock. It does the oddest thing, as I come to the bottom of a hill and start to ascend the car suddenly loses power and sometimes the check injection warning comes on. The warning doesn't always illuminate. The car is sometimes jerky when at low revs, normally in traffic. There is lots of black smoke from the exhaust when its doing its jerky/power loss thing.

If I drive a bit harder (not racing but faster than normal) the car is a lot better and does not jerk or lose power. It has to be kept above 3000rpm to run without fault. As you can imagine this is not so easy to do as the 1.5DCi is a lively little engine.

I've had the EGR cleaned which helped for about 200 miles but the problem has come back again.
